I-693: Report of Medical Examination and Vaccination Record

Thank you for inquiring about your Immigration Exam with Dr. Tanya J. Becker, an approved Civil Surgeon for USCIS , completing I-693 forms for over twenty years. The office is conveniently located in Suite 206B at 101 W. Main Street in Frisco, CO 80443. You will enter the West Main Professional building. Enter this building, take the elevator to the 2nd floor and proceed to Suite 206. Room B will be at the end of Suite 206 hallway. Please help yourself to refreshments as you enter the suite. Please note that Dr. Becker is ONLY in the office for appointment times. Dr. Becker is not in the office if there are no appointments scheduled.

This is a private office that offers a quiet and inviting space. No registration desk, no sitting in a waiting room with sick individuals - come right in and enjoy a cup of coffee or tea; water if you prefer. We have candies and snacks; special gifts for those patients under the age of 10!

What To Bring To Your Appointment

You are not required to fast for this appointment. You may eat breakfast or lunch before your appointment.
A photo identification (passport, drivers license, school ID)
Record of vaccination (see 'required vaccinations' below) If you have not received all of your vaccinations before your appointment, you may obtain them later - however, your I-693 form cannot be completed without your required vaccines. You will be given an order for any vaccinations that you have not had, or do not have documentation of, at the time of your appointment. Dr. Becker does not give immunizations in this office. Please translate any vaccinations you may have received elsewhere, into English prior to your appointment.
A translator MUST accompany you if you do not speak English. If you come without a translator, your appointment will be rescheduled.
Your fee is due at the time of your appointment. Cash only please. If you do not come to your appointment prepared to pay this fee, your appointment will be rescheduled.

General Information

PLEASE BE AWARE THAT DR. BECKER WORKS HARD TO MAKE THESE APPOINTMENTS AS CONVENIENT FOR YOU AS POSSIBLE. HOWEVER - THE IMPORTANCE OF THIS DOCUMENT SHOULD TAKE PRIORITY OVER ANYTHING ELSE SO IT IS ADVISED THAT YOU BE FLEXIBLE AS WELL WHEN ARRANGING FOR THIS APPOINTMENT AND FOR PICKING UP YOUR COMPLETED DOCUMENT.
The total cost for your Immigration Exam, excluding your vaccine cost, is $400.00. This fee will cover all required information, which includes a medical history, the results of any required laboratory tests, as well as the completion of your I-693 form, with a copy for yourself. This copy can be copied for your lawyer.
There will be an additional fee of $100 if your tuberculosis test is positive and you require a chest x-ray and medical treatment. This fee does not cover your chest x-ray. Dr. Becker will arrange for your chest x-ray and discuss any further medical treatment that you may require.
Your I-693 form will be completed as soon as the laboratory test results and vaccine record have been received. Dr. Becker will contact you as soon as your form is ready for pick up. Generally this process takes no more than one week to complete.
If you live far from Summit County and find it a hardship to return to sign and pick up your I-693 form, Dr. Becker will mail you your completed I-693 form with a copy, for an additional fee of $20.00 to cover mailing fees. Your form will be sent by certified mail.
We offer convenient days and times for your appointment, including evenings and weekends.
Your visit with Dr. Becker should take no more than one hour.
There will be an additional fee if your I-693 form needs to be redone for any reason. That fee will be determined upon assessment of the necessary changes.

*Where Do I Get My Vaccines?

Dr. Becker does not administer vaccines in the office. Your options for receiving any required vaccines that you do not have are:
Your doctors' office: You should be able to receive any USCIS-required vaccinations at your doctors office.
Pharmacy: Pharmacies such as Walgreens, City Market, Safeway and Walmart will give USCIS-required vaccinations. They may give them as a walk-in patient, or give you an appointment to return.
Your County Public Health Department: The public health department in any Colorado county may offer USCIS-required vaccinations at either no charge, or for a small fee. Please call your county health department to inquire. See the contact information below.
If you have had any of the 'required vaccinations' previously, bring this document to your appointment. Please have them translated into English if necessary.

*If you do not have a copy of your vaccine record and you're not sure what vaccines you may have had, you can receive the required vaccines at NO medical risk.

*If you have health insurance, it should cover the cost of your vaccinations if receiving them at your doctors office or through a pharmacy.

*Required Vaccinations

Tdap (Tetanus, Diptheria and Pertussis; your last dose must be within the past 10 years)
MMR (Measles, Mumps and Rubella; only if you're younger than 65)
Varicella (also known as ‘chickenpox’; if you have had 'chickenpox' as a childhood illness, you are not required to have the vaccine. A verbal history of having this illness is all that is necessary. Many times a parent or family member recalls that you have had this common childhood illness, or you may have small scars from having had chickenpox)
Flu (during flu season from October 1 - March 31)
Pneumococcal (PCV - only if you're over 65 years)
Hepatitis B (at least one initial dose if you are less than 65 years)
Polio

* If you have any medical issue for which you cannot receive one of these vaccines, or if you are pregnant, please discuss this with Dr. Becker at the time of your visit.

*Please see the 'Vaccination Requirements' link for required vaccinations for applicants 18 years and younger. Generally, if you are under 18 years of age, you are required to have those vaccinations that are required for school entrance.

*Vacunas requeridas

*Dr. Becker will collect these samples at the time of your visit.

*If any of these tests are positive, Dr. Becker will make the appropriate arrangements for treatment or referral. If your tuberculosis test is positive, there will be an additional fee of $100 for further required testing and treatment. If your syphilis test is positive, there will be an additional fee of $50 for additional required testing.

*No tests are required for children under 2 years of age.
